Neuromodulation and Women’s Health

Over the past couple of weeks, I've been researching period pain and stumbled across Neuromodulation.

A trailblazer in this space using Neuromodulation to bring relief to those struggling with dysmenorrhea is Myoovi. In Myoovi's case a gel pad is placed on the abdomen or lower back delivering small pulses to prevent the pain signals from traveling to the brain. The result? Instant pain relief according to the company and many women, if online reviews are to be believed.

I'd never really looked into Neurotechnology, so this discovery was quite intriguing and I embarked on a bit of a deep dive into the world of Neuromodulation to explore what other areas in the women's health space - besides period pain - it could be used for.

But let's start from the beginning...

👉 What is Neuromodulation?

At its core Neuromodulation refers to technologies that act directly upon nerves. It is a process that alters nerve activity by delivering electrical or pharmaceutical agents directly to a target area. The technology carries the potential to treat a variety of chronic conditions that are resistant to conventional therapies, or where such therapies carry significant side effects.

👉 How does it work?

Neuromodulation works by sending targeted electrical or chemical signals to specific neurological sites in the body. These signals can either stimulate or inhibit neural activity. This technology encompasses both non-invasive procedures such as transcutaneous electrical nerve stimulation (TENS) and more invasive methods such as deep brain stimulation (DBS) or sacral nerve stimulation (SNS), which require surgical implantation of a device.

👉 Challenges and Opportunities in Neuromodulation

While neuromodulation presents a host of opportunities, it also comes with challenges. The invasive nature of many neuromodulation therapies can pose risks, and the cost of devices can be prohibitive for some patients. However, non-invasive methods like TENS offer a less risky and more accessible option for many patients (the earlier mentioned Myoovi is an example here).

Additionally, the efficacy of these treatments can vary from person to person, and more research is needed to understand why this is the case and how these therapies can be personalized and optimized.

Despite these challenges, the opportunities in neuromodulation are significant. It can provide an alternative to drug therapies, which sometimes come with systemic side effects, and is a rapidly evolving field with advancements in technology leading to more effective and safer devices.

Most importantly, it opens a suite of new opportunities for the treatment of conditions that have historically been difficult to manage.

👉 Neuromodulation in Women's Health

So 'what about women's health 'you may ask. After researching the spaceweI've found that many conditions that primarily or exclusively affect women (and are not dysmenorrhea) are potential targets for neuromodulation therapies:

1️⃣ Endometriosis: Endometriosis is a somewhat obvious one as painful periods are a common symptom. Hormonal and surgical treatments are commonly used today, but there has been some exploration of non-invasive neuromodulation techniques like TENS for pain management in endometriosis.

2️⃣ Overactive Bladder (OAB): OAB affects about 12.6% of women in the U.S. Sacral nerve stimulation can help manage OAB. For those who are not good candidates for surgical options, non-invasive alternatives like posterior tibial nerve stimulation (PTNS) might be considered.

3️⃣ Fecal Incontinence: Affecting about 8.9% of women, fecal incontinence can be managed using sacral nerve stimulation.

4️⃣ Vaginismus: This highly stimatized condition affects a woman's ability to engage in vaginal penetration. Neuromodulation therapies like non-invasive TENS have been explored as treatment options.

5️⃣ Pelvic Organ Prolapse: Pelvic organ prolapse occurs when a pelvic organ drops from its normal place. Sacral neuromodulation, which uses electrical pulses to stimulate the sacral nerves, has been proposed as a potential treatment method.

6️⃣ Postpartum Hemorrhage: Postpartum hemorrhage is excessive bleeding following the birth of a baby. Non-invasive neuromodulation techniques like TENS have been explored as part of the management of postpartum hemorrhage.

When reviewing the above list, please keep in mind that this is an active field of research and more research is needed to better understand the challenges and opportunities.

Given the potentially disruptive nature of neuromodulation therapies, however, we didn't want to miss the opportunity to put it on your radar, especially since it isn't a topic discussed often in the context of women's health.
