We remove barriers to healthcare by offering same-day appointments and providing care based on building relationships rather than completing transactions. Each member of Zócalo Health is paired with a community health worker (promotora de salud) to establish a relationship-based partnership with a focus on understanding the member’s goals and values. Research findings have shown that relationship-based partnerships, over time, have the power to influence health outcomes. At the core of this care model is a focus on prevention, primary care, behavioral health, and traditional practices that work together-in relationship- to support our member’s wellness. Role Description

As a Community Health Worker (CHW) you will join Zócalo Health at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing. An integral member of the Zócalo Health team, the CHW will help us on our mission to deliver high-quality whole-person care that is accessible, convenient, affordable, and most importantly – culturally appropriate. You will work directly with our patients to help them navigate their local health care ecosystem, provide wellness coaching, and engage the community in the mission of Zócalo Health. You’ll be at the front end of an innovative program which incorporates community health workers and virtual care in the context of a primary care practice.

The Community Health Worker will contribute in the following ways:

  • Respond to in-bound inquiries from prospective members and move candidates through the enrollment experience
  • Be assigned to Zócalo Health members and assist with health care navigation and access to Zócalo Health services
  • Provide health and wellness coaching in alignment with the member’s care plan
  • Lead customer acquisition efforts and projects
  • Work with community leaders and local organizations to sponsor Zócalo Health events
  • Work closely with Zócalo Health’s marketing team to bring in local knowledge and insights to marketing campaigns
  • Lead and manage special projects related to Zócalo Health’s 2022 business goals
  • Regularly communicate and collaborate with product and clinical teams
  • Work alongside other care team members and primary care physicians to improve the Zócalo Health patient experience

Requirements

  • Deep commitment to improving healthcare for Latino patients
  • Fluent in English & Spanish (read, write, and speak)
  • 2-3 yrs experience as a Community Health Worker, Health Coach, Medical Assistant or related field
  • Excellent listening, communication, and interpersonal skills
  • Technologically proficient with using EMR, virtual communication and practice management tools
  • Knowledge of the local community and related health care resources
  • Strong project management and leadership skills
  • Comfortable with ambiguity and willing to incorporate innovation into your care
