Age 35 used to be considered the fertility cliff. Now it's just the beginning of a major market opportunity.
As more women delay pregnancy, egg quality has become a primary constraint in reproductive medicine - and is shaping up to become one of the largest commercial opportunity in fertility care.
The clinical data is clear: Over 70% of eggs carry genetic abnormalities by age 40, and conventional IVF fails for 80% of women in their late 30s. Despite significant investment and growing market demand, current diagnostics can only measure egg quantity, not quality - the factor that actually determines success.
Today this gap between market demand and diagnostic capability has created three distinct innovation and investment themes, each with different risk profiles and commercial timelines. And today I'd like to take a closer look at all of them.
Theme 1: The Measurement Gap
Current fertility diagnostics focus on quantity rather than quality. AMH tests measure hormone levels produced by ovarian follicles, providing insight into egg supply but no information about genetic integrity or viability. FSH levels and antral follicle counts can add clinical context, but the fundamental limitation remains the same.
The diagnostic gap has created demand for testing that provides incomplete clinical information. AMH testing costs $100-300 today and provides data that doesn't predict pregnancy success or IVF outcomes. However, demand continues to grow because it offers patients some measure of information in an otherwise very uncertain process.
Companies developing direct egg quality assessment represent the clearest market opportunity. However, there's currently little (none?) available in the direct-to-consumer space, though noteworthy examples examining oocyte quality in a clinical context are Presagen/Astec's s Life Whisperer Oocytes or Fairtility's Chloe OQ
Theme 2: Consumer Supplements
The fertility supplement market has grown significantly by providing patients with actionable steps during what would otherwise be a passive waiting period. This addresses a fundamental challenge in fertility treatment - egg health, but also the psychological difficulty of having limited control over outcomes.
Companies like Ovum, The OVA Co, or WeNatal have built businesses around this patient need. Their products typically include CoQ10, alpha-lipoic acid, and targeted vitamins that support cellular energy production and antioxidant activity.
The commercial model works because these products give patients something tangible to do to improve their chances. The 90-day egg maturation cycle creates natural subscription opportunities, as customers need consistent supplementation over several months to see any potential benefits. Success, though, is really only measured through clinical testing (see Theme 1) rather than immediate consumer-facing results, requiring patient education about realistic timelines and impact.
Theme 3: Biotech Solutions Target Fundamental Biology
Biotech companies are pursuing interventions that address the underlying causes of egg aging. These approaches require significant clinical validation but offer potential for substantial clinical and commercial impact.
Companies like Ovo Labs focus on reducing aneuploidy - chromosomal errors that increase with maternal age. Their approach integrates with existing IVF protocols and has demonstrated reduced genetic errors in eggs during preclinical studies. U-Ploid is also focused on egg health, while Gameto has developed technology to mature eggs outside the body using lab-engineered ovarian support cells.
The Integration Opportunity: Data and Personalization
What's encouraging is that companies are beginning to integrate diagnostic data, lifestyle factors, and personalized intervention strategies.
Platforms like Vie calculate fertility scores based on lifestyle, stress, and environmental factors, while companies like Doveras help parenty to-be navigate complex fertility journeys. This approach treats fertility as a multi-factor optimization problem rather than focusing on single interventions. The commercial model here shifts from product sales to ongoing optimization services.
The Reality Check
Despite the marketing claims and research investments, the scientific reality remains sobering. Current supplements can support general reproductive health but cannot reverse chromosomal damage or structural aging. At the same time the most promising biotech interventions are still years away from clinical availability.
This creates a market where patient demand far exceeds proven solutions. What this means is that companies succeed today by managing expectations while providing genuine value - whether that's psychological reassurance, supplements or incremental improvements through targeted interventions.
Ultimately, the market is moving toward a simple test: Does it actually work? And the companies with the data to back their claims will eventually be the ones to lead the pack.
